Without a good case note, there is no good prescription.
In §84–104 of the Organon there is a detailed method of case-taking. In summary: let the patient speak in their own words, and do not interrupt in between. For every symptom, write down — when it began, exactly where it is, what the sensation is like, what makes it worse, and what makes it better.
Not only the present symptoms — the history of past illnesses, the family history, the mental state, likes and dislikes, food habits, sleep and dreams — all are part of the case. In the case of children, many also note the history of the pregnancy period and the vaccination history.
The famous teaching of §6: the physician will be an ‘unprejudiced observer’. The patient's account must not be altered by one's own preconceptions; whatever was heard must be written down exactly as it was.
If you build the habit of writing symptoms in this app's treatment diary, it will be easy to master this discipline of the case note.
